I had this problem, also solved it:
https://www.pathofexile.com/forum/view-thread/3605870/page/1#p25553505 If the problem occurs on the quest version trials you have infinite tries (you don't lose the key item, for me it stayed in the thing). So this happened to me: After failing the run I interacted again with the table (or whatever it is) to start a new run and it showed the floor map, as if I already had a run going. In the bottom left corner there's an icon "Leave trials" or something. I clicked it ending the supposedly-started run. Next time I interacted with the table and started a new run it didn't crash anymore. +1 with the bug, 3rd ascension and it's crashing at the start of 3rd floor...

same issue here when going to the 2nd floor.

Same I have done this 3 times and every time it fails no matter what I do. I always get to floor 2 and get this message and nothing fixes it. Have had 4 of these keys and all 4 have been lost.... please do something about this

Lol, I'm at my fourth ascendency and the server did the same thing to me. I killed the first 3 bosses and now I can't start the new track to the fourth... Yuck!!!

Same here. I guess I will wait for the fix. Currently stuck at Trial of Time entrance. Lol
