I have the same problem, looks like if u used one time is done..

Yes, same happened and still happening.
I have gone through over 1000 runs and it did not appear.
It is either a bug or my bad luck is just unfathomable.

My recommendation for solution is, if there is an exact chance for each room to appear the chance for rooms that did not appeared for a while should get increased until you hit them.

We encountered a similar issue and tried a few things. If you kill the Architect, the Sacrificial Chamber starts appearing again

Same problem! haven't seen it, I've walked through several hundred temples. Now I'm going to double-corrupt my equipment and AFK

Yep, I'm stuck at 30 rooms after having spent ~120 temple runs looking for sac chamber. I'm probably gonna take a break until this gets hotfixed, or just quit if it doesn't. I already spent a full week just trying to get to this point, although more than half of it was being stuck at this step.
