Journey's End Instant One Shot Exploding Ship with zero warning

Unless it bugged out for you, there was also a visual indicator of some gunpowder igniting, and slowly moving towards a bunch of explosive barrels before it exploded.

I would get how someone wouldnt be paying attention to that though. thats the one and only time something like that happens in the game.

At the very least, imo it shouldnt remove you from the hardcore league (dont know if it does or not) if you die from it.
