Encountered this bug today. T13 map, rare out of bounds beyond a large log next to a cart.

Edit: not sure if it helps, but I did have delirium, Expedition and +1 level to monster on it. It also had 28% increased number of rare monsters.

Creek could not be completed, last rare mob outside north edge of map.

Large log at north edge of map goes across river and separates player from rare mob with no way of reaching mob.
