PoE 2 - Inscribed Ultimatum of the Chimera Disappeared

I lost the item as well as soon as i teleported...

my character is bricked, doing it in a group is literally impossible, gonna quit.

Yep, can't do this anymore. So I'm stuck at cruel without 2 ascendancy point RIP entire build :)

EDIT: I've completed it with my friend. He was the leader of the lobby and the map. I could click on ascendancy thing at the end after the final boss ;)

I had the Inscibed Ultimatum, went to do the chaos trial. Just before the last treial the boss fight the game kicked me out. Locked back in, I was at the begining where i need to put an offering to start the trial. But the item was gone and I couldnt start a nother trial. Went back to Utzaal to the Chaos Statue where I got it, cant get a nother one. I need help.
