Gemblood Skull did not drop

So I have been playing the game, have made it to the interlude part of the game and noticed I need more spirit to use more skills. I looked online and found that I was suppose to receive more spirit from some quests. So i went back to Act 1 Freythorn and saw that I already Killed The King of the mist but I never received an item to increase my spirit so I killed him 2 more times and still the Gemblood Skull has not dropped or is an option to take from the Ritual rewards. I then went and killed the Bog witch in Act 3 in Azak Bog she did drop the item for me to increase my spirit. I have read online that this has been an issue for other people in the past. So now I am reporting it for my self in Hopes that a solution can be found without me having to start all over on another character so I can get this extra spirit needed for future character builds.

Last 40 spirit drops from 3rd interlude when you kill Lythara, did that already?
