Paquate's Mechanism destroyed my item - multiplayer client

I just had a unique destroyed by Paquate's Mechanism which should just apply a regular Vaal Orb effect, which doesn't have a destroy roll.

I was the client (non map owner) in a multiplayer session, we'd both opened the device.
The other player used the device first, sucessful result (+1 socket) and when I hit the use button, the mechanism did its usual completion animation, and I got... nothing.
No item at all.

Update:

Item not gone permanently, the machine was uninteractable so I couldn't get it out.

The item came out of the next machine I interacted with in the Vaal temple.
