weapon swap leads to suicide

there is a bug that kills you. the chat says "Name has commited suicide". I assume it is an interaction caused by the spirit gem buff Berserk.
if anyone has an explanation please let me know. no it is not because the weapon swap leads to stat changes that then causes the death. you cannot recreate it as it only happens sometimes.
