Xopec's Medallion bugged

I took a Xopec's Medallion and potentially could store 12 crystals, but when I completed the 7th vaal beacon circle, no chest or completion show or spawn. When i went to the vaal temple to check, my cyrstal storage shows as 0/12.

to add on, when i'm in the temple, the crystal storage shows at 0/12 and when i complete the vaal beacon circle in the map, it shows as 1/6 instead of 1/12.
