Multiple issues with location connectors on World Map

Red circled locations are inaccessible despite clearly being connected to a completed location.

Blue circled location wasn't accessible after completing the tower to the east of it, but it became accessible after completing the zone just north of the tower (blue squares).

Green circled location doesn't appear to be connected to any other locations. Will update if it becomes accessible as I continue mapping.
