Possible bug in Act 4, fighting with First Herald

It's happened that dying at the same time than defeating the First Herald, resulted, after re-spawn, in the game being stuck without possibility to move forward. Tried re-spawn at checkpoint with same result.
I had to create a new instance and do Excavation site quest again.
