Missing two skill points from the campaign

I finished campaign and now I'm 69 lvl (pathfinder). I have 96 skill points now (including 6 additional skill points from the pathfinder ascendancy).

There are 24 skill points that can be obtained during the campaign, right?
The expected number of skill points should be 68(from 69lvl) + 6(from ascendancy) + 24(from campaign) = 98, so 2 points are missed.

I didn't not "touch" weapon sets points - both show 24 free points (that should mean I have all 24 from campaign, or?). Maybe I've mistaken with calculations?

I've checked from the acts and interludes what I've got and completed:

Act 1
1) The Crowbell
2) Una's Hut

Act 2
1) Kabala (Keth)
2) Final letter (the body in Deshar)

Act 3
1) Mighty Silverfist
2) Sacrificial Heart in the past

Act 4
1) Fight with the monkey
2) Do not see another 2 skill points reward, but if I'm not mistaken it was Jorney's End after Delirium fight?

Interlude 1
1) Kill Oswin

Interlude 2
1) It should be Akthi and Anundr as I remember. And I killed them for sure, but so not see a note on the map - Akthi and Anundr icon crossed, but there is no note like "Grants 2 weapon set passive skill points". Maybe this is the reason? But I checked Risu in the camp many times - there is no reward dialog.

Interlude 3
1) Kill Yeti
2) Took the last book from the Hooded one, but do not see any note about it on the map

Can you please help me with this issue. Can it be a bug, or what you can propose me to do trying to find the lost 2 skill points.
The suspicious are rewards for Omniphobia, Akthi and Anundr and The last one book in the end of campaign. But as I said I checked The Hooden One and Risu - there is no choice for the rewards.

If you need additional info, like screenshots or whatever - please let me know.
