Boss the First Herald

Playing chaos witch, the boss died from DOT damage about 1/2-1 second after I died. Can not progress the story, did not get loot, and can not get into the chamber. I tried to reload at the checkpoint but did not respawn the boss.

Restarting the level at the waypoint will reset the fight and can progress.
