Dead, no UI for "You are dead" and no respawn button

Playing on pc with a controller

Controller input
Could open/close minimap and Activate secondary skill set. Everything else blocked (obviously due to a dead state).
Could not open "escape" menu on controller in this state.

Was able to access "escape" menu on keyboard for force respawn.
