Attemp to change into human crashes the game to login screen

Problem is this:
I play druid and encountered this issue in Act 3.
First happened when I tried to enter the Viper arena. Game wanted me to change into human (I was in wyvern form) and crashed as soon as I crossed some trigger on the arena entry point (I guess?)

Then I respawned at the Utzaal last checkpoint with all item and exp progress rolled back to that checkpoint state. Huh?

Then I found out that whatever I do, if it involves changing into a human - I get same game crash.

Then I tried to unequip talisman manually - it worked. Then I ran to the bridge killing mobs again, fine so far. Now weapon swap to change into human - crash again.
It repeats again and again. Looks kinda broken, like, yeah?

Hope it can be fixed soon. Otherwise great league guys!

I am Having this same exact problem (bear form though) in the second interlude, completely unable to progress. I've updated my drivers and still have the problem.

I walked into first zone and got my cheeks clapped by a white monster wolf like character. I am now also, dcing like crazy from various bugs in different zones.
