Medalion Bug from Valley of the Titans

I don't think I've seen this bug reported. The medallion Act 2 Valley of the Titans is not allowing me to allocate which buff it gives. I am also not getting the 2nd Belt Slot the buff would allow. The only option that shows is Close.
