Well of Souls - Crafting failed

Hello,
I just tried to use the Well of Souls to reveal the desecrated modifier on my withered wand (rare).
I used the Jawbone on it and it has the green blurry text.
Then I put it in the Well and click "reveal".
The Box under the item remains black and a message in chat appears: "Crafting failed: Could not generate mod".
Was really looking forward to modifying my wand, i tried multiple attempts, restarting game, coming back later etc. nothing worked.

Same thing happens here with my atuned wand iLevel 4, with an available prefix mod slot. Perhaps the minimum desecrated mod level is not allowed for a low level item?

Same here with a rare withered wand too

I just encountered this issue on my body armour, put item in box click reveal and its all blank. Restarted game/pc and log back in to try it again and nothing happens.
