Lost my main weapon when zoning to Trial of Chaos

It happened as stated in thread title. It literally ate my main weapon when zoning to Trial of Chaos. I was there , put a Inscribed talisman with my companion and entered zone. When respawn I had no main weapon and my companion gone. Not a uber super weapon, regular two-handed hammer but with +7 melee affix.
Barely few Divs but would appreciated it very much if you can restore it to me

Might happened. However as a warrior don't have 2nd set
