Failed to connect to any instance

Whenever i try to get into a map or back to my hideout. The game kicks me out and says "Failed to connect to any instance".

I can recreate it just by teleporting back and forth just for a few times. Other than teleporting, i have no lag issues or anything which is even more weirder.
