Omen of Amelioration not working

Omen of Amelioration was active in my inventory. I often use them and always have one in my inventory.

I just died while running a map and the omen was not consumed. It also happened multiple times so far, so please fix it.

Are you a titan with the special bag? This happened to me on previous league when I stored the omen on the titan bag. I took it out and put it in the normal inventory and it worked fine that way.

I'm indeed a titan but I figured it out long before this happened so yeah, I was also holding them in my main inventory.

It's possible that another idol was triggered and cancelled this one. Also, what I learned the hard way in POE1, omens of amelioration only work once per map, the second time you die they don't trigger

Deadeye using OoA and it seems to be working correctly for me. I haven’t died twice on a map using one though so I can’t confirm or deny about the omen only working once per map. That limit isn’t called out in the text but if someone is willing to drop me two omens I’ll take two deaths in one map to test that!

Wait, You can have only one omen active in inventory at a time? Because I had both OoA and the other one that restores life/mana on low life. If thats the case....
