Omen of Resurgence not proccing correctly

On my warrior and was at low life for 15 seconds before finally dying and the omen didn't activate, captured the screen shot of it active but didn't activate when on low life for proof!

I think once you are low life, you have to survive one more hit for it to proc. Don't quote me on that though.
So you if you are at 1000 life, you get hit for 950 then you have to get hit for no more than 49 life to use the omen. |
