There's been some other threads about this, and the common theme has seemed to be people having multiple omens active at the same time. If one of them triggers, the others ones will not, the consensus seems to be that it is limited to one omen activation (of any kind) per map.

Not sure if this is your issue, but it's something worth checking out to make sure the Omen of Amelioration is the only Omen you have active when running maps.
