Mind Over Matter and Chaos Inoculation are letting damage through, failing when breaches occur

Hi, I will keep this short and sweet. For the second time, I have died to this bug. When I am doing a breach, I keep my mana full with flasks because it is my health. I get low mana sometimes, but I have a nice flask to spam. I was doing another breach today and my mana was getting low, so I spammed flask. My character was suddenly dead, with my mana being 75% full. Somehow, the game is letting at least 1 damage break through Mind Over Matter, Eldrich Battery, And Chaos Inoculation, to kill me. I imagine this isn't the way things should be going.
