Vaal soul core not registering

I'm in act 3 completing ultimatum/trial of chaos. I picked up the soul core, got the DOT as usual and the visual effect thing on my back but when I interacted with the stone altar it gave the "Missing Vaal Soul Core" message. Report is Bug Report ID: #2125841603

Was having the same problem, but I was there with a party member of mine. I picked up the core and brought it to the altar, but was unable to slot it in, it says I was missing it. Funnily enough, my party member could slot it in, even thought he wasnt the one picking it up.
The problem was, that it still showed that it was on my back and my HP was draining, but there was no place to slot it in as we completed the trial with a soul core that magically appeared on my party member. Next trial was the boss, and the Soul core was still on my back, draining my hp, even when I was talking to the trial master. I could still live next to the well, but as I entered the boss room, after my flask ran out, I died.
I literally died in the boss room before I could even touch the boss. :D
Never had this bug before and I have been playing in a party since release.
