Bug in Map Co-Op

Party members cannot rejoin a map after dying nor revive players in it, even if unused portals still exist. This has several, game-breaking implications.

- If the map creator dies, they must wait until the remaining party members clear the map before they can start another map, or the map must be forfeit. When dying early, this can lead to waiting times of dozens of minutes although there is no justification like server queues for this waiting time.

- With permanent allocation, if the map creator dies, nobody can pick up their items, ever. Found waystones are then inaccessible. At least that's true if the creator doesn't respawn.

- The overlay stating that reviving is disabled is so large, it covers up monsters and item labels, unnecessarily making things unclickable.

- The extra portals around the map device serve no purpose except selling loot and returning to the map and limiting party size (?).

In PoE 1, it worked correctly: Respawning players consume portals when rejoining. Reviving a player could have a chance to consume a portal, possibly modified by waystone affixes.

From a player perspective, it seems like all the building blocks to fix this issue are already implemented, making this a high-impact, low-effort bug, save for new affixes that modify portal consumption chances.
