Completed Iron Citadel node - did not receive Crisis Fragment

I just completed an "The Iron Citadel" node on my atlas, Ezomyte City biome.

The boss was "The Executioner" from act 1.

The node had the new white light beam in the fog, this changed to blue upon map completion.

The boss did not drop a Crisis Fragment


Also, I had a Copper Citadel ready to be run pre-patch, it has disappeared from my atlas now.
