Pausing and fog on ps5

Constant pausing. Tried changing gateway but didn't help. It pauses roughly every 2 seconds but only when outside of town, in town its fine. Can be standing still or moving. Hunting mobs or standing there trying to "wait it out", all the same result. All while having low ping. Even tried turning down graphic settings to see if it'd help, it didn't.

Fog. Clear the map, go to town, come back and map is reset, all mobs back along with the fog. And yeah I checked, there is still plenty of time left so its not from the auto reset.
