Serious error preventing ascension. PS5

I'm trying to complete the challenge of the second ascension of Act 3, the ordeal of chaos, and as soon as I kill the monsters, others are getting stuck in the scenario, preventing the challenge from continuing. Sometimes they are staying inside the walls, other times the elevator doesn't go down even though I've already killed all the monsters. I just can't get past the 3rd challenge. These errors are always occurring.
