PS4 PS5 Necropolis Sanctum Bug that make Boons duplicate rewards not work.

I have Boon Mirror of Fortune that duplicate a random offer reward.
I have choose 2 divines as reward 2 times (in Floor 3 and 4)
But when done the sanctum, it only drops 4 Divines, that means the boon not work.
I met this bug many times before but just think it was not choose the reward, but it appears many times, so this must be a bug.
