1st Kitava skip bug

When fighting Kitava for the first time and leaving the area through a portal in the moment Kitava is about to use his laser to destroy the Sign of Purity, the player becomes unable to rejoin the instance through that portal.
The error message "failed to join instance" is shown when trying to use the portal. When entering the area through the previous one, the player cannot pass the boss room entrance door. Using a portal provided by a party member who's at the docks to the next act, the player who left the area thourgh a portal is unable to see Sin and Lilly Roth. The player can enter the Cathedral Rooftop from the docks after using a friends portal but the boss door will stay closed. Using the ladder to get back down to the docks is not possible. When remaking the instance and restarting the fight with Kitava a new Sign of Purity is not needed. Toching the socket for the Sign of Purity will simply replace it, even without the item in your inventory. This bug causes the player to remake the instance and refight Kitava to be then struck by the laser, put into cruel affliction and taken away by Sin to be able to proceed with the story line. The game must lock the instance once the beam appears; normally Sin swoops down to take you away to the Docks. No loot is dropped (IIRC) so that would explain why no one would be wanting to go back.
Storyline-wise, it makes sense; Kitava just destroyed the only object capable of killing him (at the time), so you wouldn't want to go back. I think that the game executes an event once the beam appears, then a few seconds later triggers a "Kitava destroyed the Sign of Purity" flag on your character. So if you left once the event is executed, but before the flag is triggered, you're out of luck. (small act 10 spoilers)

(My other half had a similar issue during the Act 10 Kitava fight when partied with a friend; her character was killed, and in the time it took to respawn, put up auras, and return, the friend had killed Kitava, but she did not get the credit)
It doesn't make a bit of difference, guys. The balls are inert.

"Actually, I did not die. When we killed Kitava, Sin and Innocence were still hanging out but I was stuck. Kitava was dead as a doornail, I could freely leave and come back but I still had the mission "Kill Kitava" and as a result had to do it over again in a new instance whilst my friend was able to leave and keep going. It does make one wonder if the mechanics of the Kitava fight itself (regardless of Act 5, 10 or Belfry map) can accidentally do this.
